PSYCHIATRIC EMERGENCY INTAKE FORM

Here’s a psychiatric emergency intake form that uses databases to obtain diagnoses and codes.  With minor changes, it's a more general psychiatric evaluation form.

MEDICAL APPLICATIONS

Just pick the one you want, and the proper code gets inserted, too – in as many places as you want.  Here, put it in the Emergency Intake and it gets entered into the Treatment Plan at the same time.  (Poor fellow, schizophrenia AND a personality disorder AND meningitis!)

How about a mental status exam by voice?  This one is set up as a checklist, but there are other formats.

All the branches of the Axis IV tree, and the definitions for the GAF scale are available if you need them (and not if you don't).

The medications you typically prescribe are compiled in a list so   you can select them by voice (as many as you wish), and they'll appear in the labels on the next screen so you can cite dosages.  (Kindly ignore the possibility that this particular combination of medications is probably lethal - if we were physicians, do you think we'd have time to do programming?)
